Federal prosecutors say a physician with offices in Michigan City and Chicago submitted more than $3 million in false medical claims to private insurers and the government.

The U.S. attorney's office in Chicago says Paul Madison, 59, was indicted on 11 counts, including health care fraud. The charges are linked to Madison's practice at Watertower SurgiCenter in Chicago.

Madison is also medical director of Midwest Pain Clinic in Michigan City.

Prosecutors allege that Madison directed Watertower's staff to submit bills for unperformed procedures to private insurers and the U.S. Department of Labor's Office of Workers' Compensation Programs.

Each count carries a maximum penalty of 10 years in prison.
